Bloom Health Centers Patient Portal

Are you a patient of Bloom Health Centers? If so, you can now access your health information online through our secure patient portal. The portal allows you to view your medical records, schedule appointments, pay your bills, and communicate with your care team.

How to Sign Up for the Patient Portal:

To sign up for the patient portal, you will need to provide your name, date of birth, and email address. You will also need to create a username and password. Once you have created an account, you can log in to the portal at any time to access your health information.

How to Use the Patient Portal:

Once you are logged in to the patient portal, you will see a dashboard that provides an overview of your health information. From the dashboard, you can access your medical records, schedule appointments, pay your bills, and communicate with your care team.

To view your medical records, click on the “Medical Records” tab. You will be able to see a list of all of your medical records, including test results, medications, and allergies. You can also download your medical records to your computer.

To schedule an appointment, click on the “Appointments” tab. You will be able to see a list of all of your upcoming appointments. You can also schedule a new appointment by clicking on the “Schedule an Appointment” button.

To pay your bills, click on the “Billing” tab. You will be able to see a list of all of your outstanding bills. You can pay your bills online by clicking on the “Pay Now” button.

To communicate with your care team, click on the “Messages” tab. You will be able to send and receive messages from your care team. You can also request prescription refills through the messaging system.
