How to Access the Crouse Hospital Patient Portal:

Existing Users:

1. Visit the MyHealthChart login page: https://mychart.crouse.org/Crouse/Patient
2. Enter your username and password.
3. Click on “Log in.”

New Users:

1. Visit the MyHealthChart registration page: https://mychart.crouse.org/Crouse/Patient/Registration
2. Click on “Request Access Here.”
3. Enter your first initial, last name, date of birth, and validation code (which is your first initial, last name, month (2 digits), and year (4 digits) of your registration).
4. Follow the on-screen instructions to create your account.

Validation Code Example:

• Your Name: Be Healthy
• Your Visit or Registration Date: May 2018
• Validation Code: BH052018

Forgot Password:

If you have forgotten your password, click on the “Forgot Password?” link on the login page and follow the instructions to reset it.

• The Crouse Hospital patient portal is only available to current or past Emergency or Inpatient Crouse patients since March 1, 2014.
• If you have not been a patient with Crouse Hospital, you will not be able to create a MyHealthChart account.
