How to Enroll in MyLifespan Patient Portal

To enroll in MyLifespan, you will need an activation code. You can obtain this code from your healthcare provider or by completing the online enrollment process.

How to Access MyLifespan Patient Portal

Once you have enrolled in MyLifespan, you can access the portal by visiting the website: You will need to enter your username and password to log in.

MyLifespan Mobile App

The MyLifespan mobile app is available for both iOS and Android devices. The app provides the same functionality as the web portal, allowing you to access your health information, communicate with your healthcare team, and manage your care on the go.

How do I add my child to my lifespan account?

Lifespan is pleased to now offer either an in-person or remote video method to sign up for your child or teen’s MyLifespan account and proxy. An in-person sign up can be completed as part of a regular office visit or by scheduling a separate appointment for MyLifespan activation.

What EMR does lifespan use?

MyLifespan is part of LifeChart, Lifespan’s electronic health record system. LifeChart allows us to create and maintain a single electronic medical record for each of our patients.
