How to Enroll in MyLifespan:

For Lifespan patients:

1. Request your personal access code from your Lifespan healthcare provider.
2. Visit https://my.lifespan.org/mychartprd/ and click on “Sign Up for MyLifespan.”
3. Enter your access code and follow the instructions to create your account.

For Bradley Hospital patients:

1. Visit https://my.lifespan.org/mychartprd/ and click on “Sign Up for MyLifespan.”
2. Select “Bradley Hospital” from the list of facilities.
3. Follow the instructions to create your account.

Special Rules for Parents and Guardians:

Parents and guardians of children under the age of 18 have limited access to their child’s medical records through MyLifespan. To request access, visit https://my.lifespan.org/mychartprd/ and click on “Request Proxy Access.”

To access MyLifespan, you will need:

• A web browser that supports TLS 1.2 or higher
• A valid email address
